The original and one copy of the proposal and/or notification of award must accompany this form.

    With this Word file you will be able to complete/fill in, the transmittal and then save and print it for further use. This would allow you the option of customizing transmittals for each faculty member, or for each agency.

    • Space Planning for Contract & Grant Applications Transmittal Form Information
      Program and personnel planning for a grant or contract application must make explicit the implications for space and facilities support. If this section of the transmittal form is checked “yes,” note that the instructions require a written copy of the space plan to be attached as part of the application. The space plan is to be developed in consultation with the lead dean or relevant vice president’s office space representative and include three essential components. First: commentary regarding the type, amount, and when the space will be needed based on personnel, equipment, technology, and needs of specialized activities. Second, the plan must include how the unit intends to meet the space needs through reallocation within currently assigned space in the department or college or reallocated from another MAU. Finally, if alterations or improvements in space are required, a cost estimate developed by the MSU Physical Plant and a funding plan needs to be attached. Primary responsibility for developing the space plan rests with the principal investigator and must include approval of the lead dean(s) associated with the project.
      Note: It is recognized that in selected instances, the space plan may not be fully developed prior to the required submittal date. However, if the contract or grant is awarded, prior to acceptance of the award by the Board of Trustees, a plan must be in place that provides the space that may be needed, related alterations required, and includes a funding plan to address the alterations.
